• Old Passport or declaration of loss or stolen passport “Form reporting Theft/loss/deterioration” • Application form “Exit place form for adults” • Two identical photos (ICAO standard) • Declaration of absence of impediments to the issuing of the passport • Proof of Aire Registration • Most recent proof of address issued in the last month: Utility Bill, Bank statement • Cash or Bank Transfer to cover the Special Delivery costs to and from the Consulate in Manchester (the cost for this quarter is included in the appointment confirmation email). The Consulate in Manchester will contact you directly for the payment of the consular tax at the time of issuing the passport
• Old passport or the “Stolen/loss/deterioration report form” • “Adult exit form” form correctly completed • Two identical passports photos (ICAO standard) • Declaration of no impediments to the issuing of the passport • Proof of AIRE registration • Most recent proof of address, issued in the last month. Utility bills and bank statement are valid • Cash or Bank Transfer sufficient to cover the Special Delivery shipping costs to and from Manchester (the amount for this quarter can be found in the appointment confirmation email). The Manchester Consulate will contact you directly for payment of the consular fee when your passport is issued
